On February 7, 2021, the Minister of Health of the Democratic Republic of the Congo announced the outbreak of Ebola  Virus Disease (EVD) following the confirmation of a single case laboratory in North Kivu Province. The case was an elderly woman living in the Biena Health Zone. The source of the infection is still being investigated.

On January 25, 2021, a case was filed with nosebleeds. From January 25 to February 1, she reportedly received outpatient care at a local health facility. From 1 to 3 February, she was admitted to a second clinic with symptoms of weakness, dizziness, joint pain, epigastric pain, fluid, headache and shortness of breath. On February 3, a blood sample was collected for EVD testing due to his epidemiological and surviving EVD link. On the same day, he was transferred to a hospital in Katwa Health Zone (part of Butembo town) following a deterioration in his condition. He was admitted to the intensive care unit that day and died on February 4. On February 5, the body was buried in the Musienene Health Zone, yet not under safe burial practices.

On February 6, Butembo's laboratory confirmed that the case was positive with EVD by GeneXpert. On February 7, a sample was sent to a classroom in Goma, and it was tested for EVD by GeneXpert on February 8. Samples were sent to the Institut de Recherche Biomedicale for next generation to determine whether the latest Ebola case was related to a new spate of events or a resurgence of the disease that plagued the provinces of North Kivu, Ituri and South Kivu in 2018-2020.

As of 8 February 2021, a total of 117 contacts have been identified and tracked. Investigations and response activities are ongoing.
